Students who are at least 11 years old or entering the 7th grade will need to get Tdap and Meningococcal conjugate vaccines.
Students who are 16 years old or entering the 11th grade will need to get a meningococcal booster vaccine.
A copy of updated 3231 Immunization Forms will need to be turned in to Mrs. Beatrice Thurman.

will be the week of March 6-10, 2023
Copies of the following documents are required along with Application:

• Form #3231 - Immunization Record

• Form #3300-- Certificate of Vision, Hearing, Dental and Nutrition Screening

• Certified Birth Certificate

• Social Security Card

• Proof of Residence of Towns County

• Picture ID of Parent/Guardian Registering the Child

• Custody or Guardianship papers issued by the Court
