Instruction for Open Enrollment
ENROLLMENT – OCTOBER 21, 2013 thru NOVEMBER 8, 2013
READ THIS INFORMATION THOROUGHLY COULD AFFECT YOUR
Enrollment begins Monday, October 21, 2013 at 12:00 am
and ends Friday, November 8, 2013 at 5:00 pm.
Listed below are the NEW PLAN MEMBERS beginning January
1, 2014. UHC and Cigna will no longer be offered.
Options will be:
Plan Options are HRA Plans. The HMO and HDHP plans have
been eliminated. Attached is a copy of the 2014
Plan Designs along with the new rates. The new rate
will be deducted from your pay check in December for
your January 1, 2014 coverage. Unused HRA funds will
roll over to the new plans for future health expenses in
March 2014. You will also have an opportunity to
receive up to $480 per employee and $480 for spouse of
Wellness Incentives. See "Wellness Incentive"
attachment for directions.
If you or
a family member is currently receiving or anticipate the
need to continue certain covered services and/or medical
treatments beyond 2013, and the provider is not
participating in the Blue Cross Blue Shield of GA
network you may request transition of care services.
Transition of care services allows a member to continue
to obtain certain ongoing services and/or treatments
from out of network provider for an approved period of
time. You must contact Blue Cross Blue Shield customer
service no later than December 31, 2013.

which will be valid for all three of our vendors.
employee that has medical insurance MUST go online to
select your new plan option. See Attachment "Instructions for Open Enrollment." If you fail to go
online and select a new plan you will automatically be
enrolled in the HRA Bronze Plan by default. The tobacco
surcharge of $80.00 will still be applied if you or any
of your covered dependents use tobacco. Each person
will be responsible for making the changes themselves
online. Myra Underwood and Diane Adams will be
available at the central office daily to assist anyone
that does not have internet access or answer any
questions you may have.
